A collision occurred in Washington between a Black Hawk helicopter and an airplane due to ignoring warnings
On January 29, a tragic collision occurred in Washington between a Black Hawk helicopter belonging to the U.S. Army and a passenger plane. According to official investigation documents, this tragedy happened due to the pilot of the helicopter ignoring warnings and orders.
The investigation revealed that the pilot, Captain Rebecca Lobach, did not carry out the necessary actions that could have prevented the tragedy. She did not lower the altitude and did not sharply turn left as instructed.
It also became known that a problem is the lack of personnel in the airport control center. As a result, flight monitoring was being conducted by only one employee. This complicated the control of the helicopter approaching the airport for visual control but unable to maintain a safe distance from the airliner.
Captain Rebecca Lobach did not respond to the pilot and instructor's warnings and ignored the command to turn sharply left.
Furthermore, the communication equipment on board the helicopter negatively affected the situation, as it encountered communication issues. This complicated the transmission of information between the helicopter crew and controllers.
As a result of this collision, both aircraft fell into the river, and none of the participants survived. This tragedy is the consequence of serious pilot errors and other factors, such as insufficient personnel in the control center and communication problems.
